Community Support Service
Our Community Support Service is for people aged over 55 living in the North Hertfordshire District.
The service is designed to help older people maintain independence by providing support and peace of mind. It helps to ensure that vital support is available to older people at a time that is convenient to them and when it is most needed.
The Community Support Service has proved extremely valuable helping many people to continue to live independently in sheltered housing or in their own homes.
Trained staff will provide support, including:
- regular agreed visits from a Community Support Worker

liaison with other agencies on your behalf

assistance with arranging help with Hertfordshire County Council’s Health & Community Services (0300 123 4042 or www.hertsdirect.org/hcs)

help with correspondence and completing forms
-
help with acquiring adaptations for your property

support with your health and wellbeing
-
assistance with reporting repairs, and dealing with rent and tenancy issues

support with debt management and paying bills.
How do I access the service?
If you live in your own home and are interested in receiving the service, please call 01462 704110 and we will arrange for a Community Support Worker to visit you at your home at a time that is convenient to you.
If you are living in a sheltered scheme this service will be discussed with you as part of your sign –up process.
You can choose to have a family member or friend with you for support.
The assessment process
Following registration, an initial Needs and Risk Assessment will be carried out to determine your support needs; the assessment balances the promotion of independence with effective risk management.
The assessment will cover areas such as health, mobility, managing your finances and adaptations, and other identified risk that may affect you living independently.

We work directly with you so that an agreement can be reached as to how best the service can support your individual needs; yearly reviews are carried out to make sure that your requirements are up to date and relevant to you. You can always request a review following a change in your circumstances.
Is there a cost involved?
There is a weekly charge for this service, however if you receive benefits then the service may cost you nothing. For further information please contact the Supported Housing Team on 01462 704110.
